You can do this yourself online, or if you prefer, we can do it together over a call where I ask you the questions and complete the form with you. The call option is better if you prefer speaking to someone directly, if you feel overwhelmed by forms, or if you have difficulty completing it on your own. The call option costs slightly more to reflect the additional time involved.

How It Works
Step 1: Complete the diagnostic form. After payment, you receive a link to a detailed questionnaire covering your symptoms, your history, what you have tried, and what areas of your life are affected. Most people complete it in about 30 minutes. If you prefer, you can book a call instead and we will go through the questions together. The outcome is the same either way.
Step 2: We review your case. Your answers are reviewed carefully to identify patterns, priorities, and the right treatment approach for your situation. Every case is assessed individually.
Step 3: You receive your response. Your written case review, or your review and full treatment plan, is delivered by email within 24 to 72 hours. You have a clear record you can refer back to and follow.
Before You Begin
You do not need to prepare anything complicated. You will need:
